The International Certificate of Vaccination (ICV) also known as the “yellow card” is a document by the World Health Organization (WHO) in the form of a booklet that is commonly used for travelers to proof their vaccination against diseases such as malaria and yellow fever. The Bureau of Quarantine in Philippines recently included vaccination proof against Covid-19 in the ICV. This is recognized by the member countries of the World Health Organization (WHO).
If you are a fully vaccinated student going to further your studies aboard or an Overseas Filipino Worker (OFW), this yellow card will come in handy for you to proof your Covid-19 vaccination at your destination country. However, do your due diligence to check with the authority in the country if they do recognize the ICV.
Step-by-Step Guide
Step 1
- Visit Bureau of Quarantine’s website at icv.boq.ph
- Select the location where you will collect the certificate
Step 2
- Click on BOOK AN APPOINTMENT button
- A Terms & Privacy Policy notice will appear
- Click on the Agree button at the bottom once you have read the policy
Step 3
- Key in and submit your valid email address
Step 4
- Next, click on the dropdown menu under Service Category and select Certification
Step 5
- Under Certification Services, click on “International Certification of Vaccine for COVID-19”
Step 6
- You will then see calendar where you can pick your preferred appointment date
- Green indicates that there are still slots available for the date
Step 7
- Click on your preferred date and you will see the slots available for each hour of the date
Step 8
- Fill-up your personal and contact information, and click Next
Step 9
- Then, key in your Valid ID, Passport, and COVID-19 vaccine card issued by the LGU or other administering center
- You will need to upload images of the requirements above including the front and back of the vaccine card
- You will see the screen below once your requirements are successfully uploaded
Step 10
- You may then proceed to pay with the following payment options:
- Credit/Debit card
- Cash (7 Eleven, LBC, Family Mart, Cebuana Lhuillier, etc)
- eMoney (GCash, PayMaya, Cliqq)
- Online Banking (BancNet, BPI, Security Bank)
- The total amount at time of writing is Php370 (Php300 for the ICV plus Php70 convenience fee)
Step 11
- Once you’re done with the payment, you will see on your screen the confirmation. You may view the receipt and download the pdf file
- Save your Reference Number for your appointment day
And Done!!
- On the day of appointment, make sure to arrive at least 15 minutes earlier
- Bring along your Valid ID and Vaccination Card
- Be sure to put on appropriate attires – shorts, open shoes/slip-ons, and sleeveless/sandos are not allowed
